From memory, I've had 3x Royal Flushes, although may have been more as been playing several years.
The ones I recall are:
1) Cash table - Doubled stack up
2) Tournament - Everyone else folded so didn't get paid
3) I remember misclicking fold once, but again, no action so guessing not lost much
Have folded several hands where it subsequently went on to hit a royal, but of course those don't count.
I tend to prefer Quads or lower straight flush personally, more chance of getting paid off imo.

My first was only a couple of weeks ago on Titan. Unfortunately it was on the .01/.02 NL tables but I did manage to double up to $4 whole dollars ♦
I've had three: one in live play against my father (not for money). I had the ace of 
and K :heart:Q :heart:J :heart:T came on the board.
Then I had one on Mansion in an SnG, but everyone folded, and one on Titan in a cash game, where I won a middle sized pot, nothing fancy. I guess it's not what hand you got, it's how you play it...
I've had three so far. All on partypoker. One at NL cash, FL cash and NL sit n go. Think Im due for another one pretty soon though.
I've been playing online poker for 2-3 years and the best I had was a K high straight flush. I'm much luckier with quads though, usually get at least one a day. Once I was dealt them in consecutive hands (I play a lot of 5-card) the chance for being dealt is about (4100 : 1) I believe.
I've had 2, one in a NLHE tournament, I had Q♦ J♦, the flop came K♦ K♠ Q♠ everyone checked the flop, then on the turn an A♦ came up, someone bet about half the pot n I called hoping for a flush or straight, then T♦ came up on the river 
my opponent bet the pot, n being a beginner I just minraised n they called with a full house KT.
The 2nd royal flush I had A♥K♥xx in PLO10 and flopped T♥ J Q♥, I checked the flop n no one bet, so I bet on the turn n everyone folded, I won 40c

Had one royal flush, got it on titan poker NL5 BSS i think, didn't get payed off :(.
I had like 1 (Spade ♦) Royal flush. It was on partypoker! And i dont remember the limit ore what strategy! ♦
And i used my both holecards!

I got three, one happened several days ago while I was playing HU, however my opponent folded, so didn't get paid much.
Btw, all was on PartyPoker as it is my home. ♦
1x royal. Hold em, at Pacific. Wen't all in on the flop with the open ended str flush draw. Beautiful river card followed...
none, on my whole life never never , the closest was a AdKdxx folded on omaha when the board paired , I would have gotten a backdoor royal flush,
