Have seen video 1 of Game Plan, but I do not understand how you set up a hand range for the opponent since PT4 only shows the opponents hands at showdown.

There is also a discussion thread that may shed some light on your question.
In the article it appears that you start with estimating you opponent's standard range, and building a range to combat. I just skimmed the article, perhaps there is more information available if it is read carefully. There are also pre-built ranges for a "standard" opponent -- but most opponents are somewhat different -- they may be tighter, looser, or they may limp instead of raising. Standard play at one limit might not be standard at another.

the pokertracker shows VPIP and PFR from each position
you look at that and there you go you have a handrange for open raising and calling
if he is in first position UG ... he open raises 11% then his hands are aprox 22+, AJ+,KQ+
