It looks like you successfully logged in.
Click the Help button, then on the left-hand side choose "contact us"
Click the button with the same name on the right-hand page, and fill in the form.
If you get the option for live chat, use it -- PokerStars live chat operators are real people, not bots -- at least I got a real person earlier today.
(it might have been a very, very sophisticated bot though 
)
