Well I know Titan has a team of four dudes throwing packs of cards on the ground and using a "random" selection of the lowest and highest face up ones to give to the best players, and the most connected and/or matching cards to give to the worst players.

I think poker rooms deal crazy cards (gutshots, flushes etc.) in order to maximize hand value and thus their rakes. They are con artists, frauds par excellence.
