The definitions for all stats are available by Clicking Configure and then selecting Stats in the PokerTracker 4 menu.
4Bet% is the frequency of 4Betting. This is going to sound counter-intuitive, but if your opponent is 4Betting 7% of the time, that does not mean that player's range is 7% of hands, it only means that the player is 4Betting 7% of the time, and nothing more than that. Its true that there is an indirect correlation to range, but it is not a 1:1 ratio. Using the 4Bet % as a starting point you can estimate your opponent's range range fairly close, but it the important thing to take away is that a 7% 4Bet probability is a measurement of frequency, not range. You are right to assume that we need to know the # of opportunities as well as the frequency of actions to calculate stats, this is the difference between a stat and a note; a note is a measurement of actions where we cannot also measure the opportunities to take that action.
4Bet+ Ratio is different, this stat shows what percentage of all hands a player is willing to 4+Bet with, if he or she already raised (2Bet). This ratio is calculated using the formula (PFR * 4Bet+) / 100. For example an opponent with a PFR of 20 and a 4Bet+ of 5 would be calculated as (20 * 5) / 100 = 1. Thus this player has been shown to 4+Bet 1% of his hands.
